Prediction of hemorrhagic complications after simultaneous dental implantation in elderly patients with cardiovascular pathology

Summary
In the study of 135 patients, the article developed a system for individual prediction of hemorrhagic complications after simultaneous dental implantation in elderly patients with cardiovascular pathology. The comorbid status of patients was characterized by the CIRS-G (Cumulative Illness Rating Scale for Geriatrics) questionnaire, which takes into account the age of patients and the specifics of elderly diseases. The total number of hemorrhagic complications during dental implantation in the clinical group was 16 (11,9 %) people. The relative risk of hemorrhagic complications when the final severity index of comorbid diseases was higher than two inclusive was 1,6 (p=0,0002), the combined intake of antiplatelet agents and anticoagulants was 2,9 (p<0,001). With a decrease in blood thrombocrit below 0,25 %, the risk of bleeding during dental implantation increased 3,1 times (p<0,001). Based on the results of the study, a prognostic model for assessing the risk of hemorrhagic complications of simultaneous dental implantation was developed. It was concluded that in elderly patients with cardiovascular pathology and a high risk of dental bleeding, antiplatelet and anticoagulant therapy should be discontinued three days before and 24 hours after dental surgery.
